He is survived by his father, Jerry Dale Sr. of Cleveland, TN; mother, Gloria Chavez Velasquez of Reidsville; sisters, Jessica Neal of Dallas, Maudie Hamilton of Cleveland, TN; 2 nieces, Tess Martin and Taylor Neal.
